Performance Insurance and Financial Services, LLC (Performance) recognizes that your privacy and the protection of your personal information is important to you. This statement discloses how we use the personal information stored in the Performance Insurance web site (www.performanceinsurance.com) and how we protect it. It does not apply to other online or offline Performance Insurance sites, products or services.

Performance Insurance is a service that allows its users to request insurance quotes from an independent insurance agent. Users consist of members of the general public in homes and businesses throughout the United States.

What Information Is Collected by Performance Insurance

When you request a quote online through the Performance web site, you may be asked to provide certain personal information that will be stored in Performance electronic systems or physical facilities. That information may include the following:

2. Your date of birth

3. Your Social Security Number

4. Your current phone number

5. Your e-mail address

6. Your driver license number

7. Information describing your autos, home, business and other property

PERFORMANCE'S USE OF YOUR PERSONAL INFORMATION

Your personal information will be used to obtain insurance quotes on your behalf. Your information will not be sold, rented or shared except in accordance with the General Disclosure section below or as necessary to perform the services you have specifically requested. In particular:
 

• Your information may be shared with an insurance carrier for the purpose of providing you with a quote

• The insurance carrier may access your credit information to determine your credit-worthiness and for rating purposes

Performance Insurance will not use or share your personal information in a manner that differs from what is described in this Privacy Statement without your prior consent.

Performance Insurance may send you e-mail about periodic service or recommended security updates related to Performance Insurance. These periodic e-mails are considered essential to the provision of the service you have requested. You cannot unsubscribe from these mailings. An example would be a notification telling you that your insurance is near expiration and that you you will not be covered unless it is renewed.

PERFORMANCE'S GENERAL DISCLOSURE OF PERSONAL INFORMATION

Performance Insurance occasionally hires other companies to provide limited services on our behalf, such as the mailing of product-related notices. Performance will only provide these companies with those pieces of your personal information needed to perform their services - such as your name and mailing address - and the companies and their employees are prohibited from using that personal information for any other purpose.

SECURITY AND STORAGE OF YOUR PERSONAL INFORMATION

Electronic Information
Your Performance Insurance information is stored on secure Performance servers located in controlled facilities. To access your information electronically, where available, you must type the correct Security Code into our web site. It is your responsibility to ensure the security of your Performance Security Code and not to share this information. If you choose to reveal your Security Code to a third-party, Performance Insurance is unable to guarantee the security of your Performance-stored information.

Whenever it is necessary to electronically transmit your information, Performance will use industry-standard security technologies to encrypt it for secure transmission.

Personal information collected by Performance Insurance will be stored and processed in the United States only.

Paper-based Information
Where your personal information exists in a paper format, it will be stored in a secure facility within a secure vault or lock-box during the retention period. At the end of the retention period, it will be cross-shredded and tendered to a document destruction company. Your information will be handled in accordance with GLBA Safeguard Rules and the FTC Disposal Rule.

USE OF COOKIES

A cookie is a very small text file that a Web site saves to your computer's hard disk to store information that you provide about yourself or to store your preferences. Any use of cookies by Performance Insurance is for the express purpose of managing session information and maintaining data security. We do not allow third-party cookies nor do we use cookies to track your activity.

CHANGES TO THIS PRIVACY STATEMENT

Performance Insurance will occasionally update this Privacy Statement. When we do, we will also revise the "last updated" date at the bottom of the Privacy Statement. Performance Insurance will obtain your consent for any updates to this Privacy Statement that expand the sharing or use of your personal information in ways not disclosed in this Privacy Statement.
